BACKGROUND AND OBJECTIVE: Children born small for gestational age (SGA) show higher risk of neurodevelopmental and cognitive abnormalities. The objective of this study is to determine in children born SGA the neurodevelopment during the first 2 years of life and to establish the influence of anthropometric data, gestational age, multiple gestation and perinatal factors. PATIENTS AND METHOD: Observational, prospective, descriptive and analytical study of the neurocognitive assessment performed, with Brunet-Lézine test, on SGA children (n = 91) from 3 to 24 months of age, comparing with own controls. RESULTS: Ninety-one SGA children, 47% girls, 83.5% single pregnancies; mean gestational age 37.7 weeks (standard deviation (SD) 2.1). Weight at birth 2053 g (SD 433.1), length 43.9 cm (SD 2.6) and head circumference 31.7 cm (SD 1.7). The SGA population shows significantly lower neurodevelopment than the control population, with a tendency to improve during the first 2 years of life. There are no differences by sex. SGA children born to multiple gestations have lower neurodevelopment only during the first year of life. There is a direct and positive correlation between weight, length and head circumference with neurocognitive development at 6, 9, 12 and 18 months. Gestational age correlated with better neurodevelopment at 3 and 6 months. CONCLUSIONS: Children born SGA present lower neurodevelopment than the control population. A greater impact on weight, length, and head circumference at birth is correlated with poorer neurocognitive development. Multiparity does not show significant influence on neurodevelopment evolution.

ntroducción: La otitis media aguda (OMA) es una de las causas más frecuentes de consulta y prescripción antibiótica en pediatría. El objetivo de este trabajo es valorar la prescripción antibiótica diferida en OMA en pediatría en atención primaria. Material y métodos: Estudio observacional, retrospectivo, mediante revisión de historias clínicas en un centro de salud urbano de la Comunidad Autónoma de Aragón de niños con OMA de tres cupos en los que los facultativos realizan prescripción diferida. Mediante regresión logística se analizan las variables posiblemente relacionadas con la prescripción antibiótica. Resultados: Se analizan 1.390 episodios de OMA en 696 pacientes. Se realizó prescripción inmediata de antibióticos en el 67,6% (IC 95% 65,1-70,0) de episodios, sintomática exclusiva en el 13,7% (IC 95% 11,9-15,6) y antibiótica diferida en el 18,7% (IC 95% 16,7-20,8), dispensándose finalmente el antimicrobiano en el 53,5% (IC 95% 47,4-59,5) de estos últimos. Factores relacionados significativamente con la adquisición final antimicrobiana en prescripción diferida son la edad entre 0-2 años (OR 1,89; IC 95% 1,25-2,87), la bilateralidad (OR 2,54; IC 95% 1,48-4,35), la otalgia (OR 0,49; IC 95% 0,29-0,82), la fiebre (OR 2,67; IC 95% 1,95-3,65), el abombamiento (OR 3,63; IC 95% 2,50-5,29) y la otorrea (OR 25,98; IC 95% 12,75-52,92). Los mismos factores influyen en la prescripción global de antibióticos. Se indicó principalmente amoxicilina (74,6%), seguido de amoxicilina-clavulánico (17,0%).Conclusiones: La prescripción diferida en OMA parece útil para disminuir el consumo de antibióticos, siendo necesario avanzar en su implantación. Amoxicillin (74.6%) is the most indicated antibiotic, followed by amoxicillin–clavulanic (17.0%).Conclusions: Delayed prescription in AOM seems to be useful to reduce antibiotics consumption, being necessary to advance in its implantation. INTRODUCTION: Short stature is the most frequent reason for consultation in Pediatric Endocrinology consultations and sometimes requires treatment with growth hormone. The aim of the study was to analyze the response to treatment based on its onset in pubertal or prepubertal stages and to analyze the possible benefit of an early onset. PATIENTS AND METHODS: Longitudinal, retrospective and observational study in 139 patients treated for idiopathic growth hormone deficiency up to adult height. MAIN VARIABLES STUDIED: (a) genetic background: maternal, paternal and genetic height; (b) perinatal history; (c) anthropometry during follow-up and at pubertal onset: weight, height, body mass index; (d) variables during follow-up and at pubertal onset: growth rate, bone age and growth prognosis. Final response variables: adult height, adult height with respect to target height, adult height with respect to initial growth prediction, adult height with respect to initial height at the start of treatment and adult height with respect to height at pubertal onset. RESULTS: Total pubertal gain was 0.84 ± 0.6 SD. 61.9% of the patients started treatment with rhGH in prepuberty. The initiation of treatment in the prepubertal stage and a higher total pubertal gain are correlated with a better final height (P = 0.001 and r = 0.507, P = 0.00, respectively). Furthermore, a longer duration of treatment in pre-puberty is correlated with a better final response (r = 0.328, P = 0.00). CONCLUSIONS: The start of treatment in the prepubertal stage and its longer duration during this period are determining factors to achieve a good long-term response. Total pubertal gain was greater in patients who started treatment in the pubertal stage.

Introducción: La talla baja es motivo de consulta frecuente en Endocrinología Pediátrica, precisando en ocasiones tratamiento con hormona del crecimiento (GH). El objetivo del estudio fue analizar la respuesta al tratamiento en función de su inicio en la etapa puberal o prepuberal y analizar el posible beneficio de un comienzo precoz. Pacientes y métodos: Estudio longitudinal, retrospectivo y observacional en 139 pacientes tratados por déficit de GH idiopático (grave o parcial) hasta talla adulta. Principales variables estudiadas: a) antecedentes familiares: talla materna, paterna y genética; b) antecedentes perinatales; c) antropometría durante el seguimiento y al inicio puberal: peso, talla, índice de masa corporal, y d) variables durante el seguimiento y al inicio puberal: velocidad de crecimiento, edad ósea y pronóstico de crecimiento. Variables de respuesta final: talla adulta, talla adulta respecto a talla genética, talla adulta respecto al pronóstico de crecimiento inicial, talla adulta respecto a talla al inicio del tratamiento y talla adulta respecto a talla al inicio puberal. Resultados: La ganancia puberal total fue de 0,84±0,6 DE. Un 61,9% de los pacientes iniciaron tratamiento con GH en prepubertad. El inicio del tratamiento en la etapa prepuberal y una mayor ganancia puberal total se relacionaron con una mejor talla final (p=0,001, y r=0,507, p=0,00 respectivamente). Además, una mayor duración del tratamiento en la prepubertad se correlacionó con una mejor respuesta final (r=0,328, p=0,00). Conclusiones: El inicio del tratamiento en la prepubertad y una mayor duración durante este periodo son factores determinantes para alcanzar una mejor respuesta a largo plazo. La ganancia puberal total fue mayor en los pacientes que iniciaron el tratamiento en etapa puberal. (AU)

Most publications on pediatric COVID-19 in Spain were performed at the beginning of the pandemic when some diagnostic tools were not widely available. This study aims to show the real spectrum of the infection based on wide detection of cases due to symptoms and contact tracing. A descriptive and analytical observational study was performed including pediatric cases (0 to 14 years) from the region of Aragón between May 12 and October 31, 2020. Diagnostics was by PCR detection of viral RNA, rapid antigen detection test, or positive IgG serology. There were 5933 positive children included. Of them, 49.03% were women. The mean age was 7.53 ± 4.28 years. The source of infection could not be determined in 17.8% of cases. As for the rest, was determined to be within the family environment in 67.8%. The percentage of asymptomatic patients was 50.3%. Among symptomatic patients, fever (58.1%) and cough (46.7%) were the most frequent symptoms. Hospitalization was required in 0.52% of infected, intensive care unit admission was on 0.05%, and there was one death (0.02%). Children under the age of one presented some symptoms more frequently (71.6% vs 48.5%; OR 2.68; 95% CI 2.08 to 3.45; p < 0.001) and required more hospitalizations (3.9% vs 0.34%; OR 11.52; 95% CI 5.65 to 23.52; p < 0.001).Conclusion: In our environment, SARS-CoV-2 infection is like other mild respiratory viral infections in the population under the age of 15. The contagion occurs mainly in the family environment, the number of asymptomatic is high, being the symptoms mild and the complications very infrequent. What is Known: • Pediatric infection produced by SARS-CoV-2 has manifested as a mild disease in relation to adult age, although with higher affectation at the youngest ages. • Nearly all studies on epidemiology and clinical spectrum of the disease were conducted with patients diagnosed at the beginning of the pandemic. By then, diagnostic tools were only available in hospitals and in emergency units. What is New: • Once diagnostic means were available in primary care medicine and were used not only for the diagnosis of clinical symptoms of the patient, but for the tracing of case contacts, a much more precise approach to the epidemiology and clinical manifestations of the disease was allowed, as described in this study.

La entidad llamada tick-borne lymphadenopathy, también conocida como dermacentor-borne necrosis eritema lymphadenopathy, se incluye dentro del diagnóstico diferencial de enfermedades transmitidas por garrapatas, que han aumentado considerablemente en los últimos años debido a una mayor observación clínica-epidemiológica y mejoría diagnóstica. Es una zoonosis causada por Rickettsia slovaca y transmitida por la picadura de la garrapata Dermacentor marginatus. Los pacientes afectados tienen una escara necrótica rodeada de un halo eritematoso en el cuero cabelludo y adenopatías regionales dolorosas. Se presenta a un varón de siete años que mostró, tras la extracción de una garrapata, una escara necrótica en el cuero cabelludo, con adenopatías y febrícula. La reacción en cadena de la polimerasa y el cultivo de la lesión confirmaron la presencia de Rickettsia slovaca. El paciente recibió azitromicina oral con buena respuesta. En Atención Primaria, es importante el seguimiento de las picaduras de garrapatas para detectar posibles enfermedades transmitidas por estas.

The entity called tick-borne lymphadenopathy, also known as dermacentor-borne necrosis eritema lymphadenopathy, is included in the differential diagnosis of diseases transmitted by ticks, which have increased considerably in recent years due to greater clinical-epidemiological observation and diagnostic improvement. It is a zoonosis caused by Rickettsia slovaca and transmitted by the bite of the Dermacentor marginatus tick. Affected patients have a necrotic eschar surrounded by an erythematous halo in the scalp, as well as painful regional lymphadenopathy. Chickenpox is not common in the first year of life (infant varicella) and there is a lack of data on its presentation, especially in primary care. A year-long observational study (July 2015-2016) carried out by a research network of primary care pediatricians throughout Spain.Two hundred and sixty-four pediatricians gathered data from 358 cases of clinically diagnosed chickenpox in infants. The illness was considered mild in 78% of infants < 7 months compared to 65% in those aged 7 to 12 months (p = 0.0144). Fever (46%) was present in 35% of children ≤ 6 months compared to 55% in older children (p = 0.0005). The number of skin lesions was > 50 in 35% of children ≤ 6 months old compared to 47% in > 7 months (p = 0.0273). From the 2% of hospitalized children 86% were younger than 7 months. Oral antiviral treatment was given in 33% of cases ≤ 6 months compared to 18% in older patients (p = 0.0023). Doubts about administering the chickenpox vaccine at a later date were expressed by 18% of pediatricians.Conclusion: Chickenpox is considered benign, having a mild effect on most infants. There is less clinical effect in infants ≤ 6 months although this age group is hospitalized more and is prescribed more antiviral treatment. There are doubts among pediatricians about the subsequent need for vaccination. What is Known: • Chickenpox is uncommon and of uncertain evolution in the first year of life • Hospital admissions for chickenpox are more frequent in the first year of life What is New: • The course of chickenpox in the first year of life is mild, especially in infants younger than 7 months despite the fact they are hospitalized more and are treated more frequently with antivirals. Antivirals are prescribed to 1 in 4 children with chickenpox under 12 months of age. • Almost 50% of pediatricians recommend a subsequent vaccination against chickenpox especially if it occurs in the first 6 months of life.

Objetivo: Conocer las principales características clínicas y epidemiológicas de la gastroenteritis bacteriana pediátrica en nuestro medio. Pacientes y métodos: Estudio observacional en el ámbito de la población española. Recogida de encuestas durante un año de niños con coprocultivo positivo a bacterias. Análisis bivariado y 2 modelos multivariantes (para las variables tratamiento antibiótico, y comparación Campylobacter/Salmonella). Resultados: Un total de 729 episodios de gastroenteritis bacteriana en las 17 comunidades autónomas (41,2% mujeres y 58,8% varones). La mediana de la edad fue 3,41 años (rango intercuartílico 1,55 a 6,72). El 59,9% de los aislamientos fueron Campylobacter, el 31,8% Salmonella no tifoidea, el 2,7% Aeromonas, el 2,5% Yersinia y más de un germen el 1,5%. La mayoría de contagios (70%) son directos, y la intoxicación alimentaria es más improbable (25,9%). Salmonella es significativamente menos frecuente que Campylobacter en menores de 3 años (OR ajustada: 0,61; IC95%: 0,43 a 0,86; p = 0,005), y Campylobacter es más habitual en el medio rural (OR ajustada 1,48; IC95%: 1,07 a 2,07; p = 0,012). Se indicó antibiótico en el 33,2% de los casos, significativamente más si hubo productos patológicos en heces (OR ajustada: 1,53; IC95%: 1,04 a 2,27; p=0,031), duró más de 7 días (OR ajustada: 2,81; IC95%: 2,01 a 3,93; p<0,000), o se hospitalizó (OR ajustada: 1,95; IC95%: 1,08 a 3,52; p=0,027). Conclusiones: La etiología de la diarrea bacteriana pediátrica es la propia de un país desarrollado. El mecanismo de contagio es principalmente directo, y se tratan con antibióticos más casos de los que parecería recomendable (AU)

Salmonella is significantly less frequent than Campylobacter in children under the age of 3 years (adjusted OR 0.61; 95%CI: 0.43 to 0.86; P=.005), and Campylobacter is more frequent in rural areas (adjusted OR 1.48; 95%CI: 1.07 to 2.07; P=.012). Antibiotic was prescribed in 33.2% of cases. There was a greater significant difference if stools contained blood or mucus (adjusted OR 1.53; 95% CI: 1.04 to 2.27; P = .031), if the symptoms lasted more than 7days (adjusted OR 2.81; 95% CI: 2.01 to 3.93; P < .000), or if the child was admitted to hospital (adjusted OR 1.95; 95% CI: 1.08 to 3.52; P = .027). Conclusions: The aetiology of bacterial diarrhoea in paediatrics is typical of that of a developed country. The transmission mechanism is mainly direct, and more cases than appropriate are treated with antibiotics (AU)
